Sensitive detection of recurrent colorectal cancer (CRC) by measuring circulating tumor DNA (ctDNA) can increase the chance of a cure. This study compared the quantitative methylated ctDNA test with carcinoembryonic antigen (CEA) in the supervision arrangements for recurrence.Blood samples collected either for surveillance or within 12 months of confirmation of recurrence was tested for ctDNA (methylated branched-chain amino acid transaminase 1 [BCAT1] / Ikaros 1 family of zinc-finger proteins [IKZF1]) and CEA.
CtDNA optimal threshold is determined by receiver operating characteristic analysis, and performance tests for the detection of recurrence compared with CEA (5 threshold ng / mL) .the study cohort comprised 144 patients were eligible and including 50 recurrence events.
Sensitivity tests ctDNA are methylated to recurrence was 66.0% (95% confidence interval [CI], 57.1% -69.3%), which was significantly higher than the sensitivity of CEA (31.9%; 95% CI, 22.8% -36.6%; P <0.001). Sensitivity for inoperable recurrence (n = 20) was also higher (ctDNA, 60.0%; CEA, 20.0%; P = 0.01). specificity did not differ between tests (ctDNA, 97.9%; 95% CI, 93.2% -99.6%; CEA, 96.4%; 95% CI, 91.4% -99.0%).
When adjusting for other predictors of the presence of recurrence, ctDNA tests positive is an independent predictor (odds ratio, 155.7; 95% CI, 17.9 to 1360.6; P <0.001), whereas CEA did not (odds ratio, 2, 5; 95% CI, 0.3 to 20.6; P = .407) .the quantitative ctDNA test indicate superior sensitivity as compared with CEA without differences in specificity for detecting recurrent CRC. Longitudinal studies are guaranteed to further assess the utility (especially the survival benefit) of alcohol BCAT1 / IKZF1 ctDNA in surveillance of patients with CRC.

In this study, we developed the von Hippel-Lindau-cereblon (VHL-CRBN) heterodimerizing CRBN PROTACs which induces degradation, but not VHL. A quantitative proteomic analysis further revealed that VHL-CRBN heterodimerizing PROTACs CRBN induced degradation, but not well-known immunomodulatory drug (IMiD) neo-substrate, IKAROS family of zinc finger 1 (IKZF1) and -3 (IZKF3).
